I guess a big question would be how far do you want to go to avoid PC death. A critical hit can kill a PC without the PC making any sort of saving throw, for instance. I've found that crits cause as much character death as save-or-die does in my campaign.
Some campaigns should indeed eliminate save-or-die effects (and perhaps crits), or perhaps limit them to the BBEG's of the campaign. Then again, other campaigns are different, so I'm glad such effects are still in the rules.
Anyone who demands to play "straight out of the book", then demands that the book match his play preferences exactly, is asking for too much in my opinion! A DM can easily declare that certain spells don't exist in his world, or the players can (less easily) push the DM to declare them!
