For the most part the ph assumes that a character with a 2handed weapon is always holding it 2 handed. But for disarm attempts, there's a couple of situations where he might be holding it only in one hand, and thus losing a big advantage.
1) You are using improved trip on your friendly neighborhood greatsword user. You trip him and then as your extra attack make a disarm attempt. Is the weapon in both hands still, so the guy just lands flat on his face or his back?
2) The guy is picking up his weapon. Is he using both hands at once?
**I'm mostly positive this one is yes, as it still makes sense either way.
3) you ready an action to hit the guy as he's standing up. As he's standing up, one handed or two handed?
