Sneak Attack, Ranged Attacks, and Reach

Can you only sneak attack with a range weapon if the target is flat-footed and within 30ft?

In other words I don't think you can flank with a bow so therefore you can't sneak attack with it, even if you are within 30 ft, unless the target is flatfooted. Does this sound right?
Interestingly, this used to be one of the big debate points, and AFAIK the only one we have ever had a breakthrough with. Gives us hope for the rest... :D

Flat-footed... or the target is denied his DEX bonus to AC, you can add SA damage to a ranged attack.

Which, strangely, although otherwise identical in all other ways, are different situations, and therefore some effects work only when flat-footed, but not when denied Dex, or when denied Dex, but not flat-footed.

Right. Being flat-footed isn't sufficient to permit a sneak attack. Being denied Dex is... and in most cases, a flat-footed opponent is denied his Dex. But if he has Uncanny Dodge, for example, he can be the flat-footedest opponent ever, and you still can't sneak attack him.
