As to the question of cover I tend to give the opponent the +2AC when the attacker is just popping his head out to shoot. I look at it as you're moving so fast you don't have time to aim properly. That assumes they want to keep their +5AC, if they don't then maybe no cover applies.
Again, all situational. There are too many variables to only apply rules one way every time.

Prception vs. Stealth not rely on sight? How does not being able to see clearly the hiding creature not imposes a disadvantage on perception checks from light obscurement ?

Because you can't hide from a creature that can see you clearly. No contest is necessary in that situation. You don't have a contest anyway to find out if the creature sees you clearly or not. No, you are simply not hidden.

You can only hide from a creature that can't see you clearly. That's the point at which there can be a contest between your Dexterity check and a creature's Perception score. The question of whether you can be seen clearly needs to be answered before that can happen. The DM decides whether conditions are appropriate for you to not be seen clearly, not the result of a contest.

Yes, that's correct. Why is that funny? You wouldn't be able to see the hidden creature anyway. It could only hide because it was not seen clearly by you.
 

Yeah, I thought about that a bit more.. The whole keen sense thing I forgot about.. wolves, dogs, cloud giants etc all have keen hearing or smell... So that would help them detect nearby invisible creatures or creatures Hiding behind that tree.. Would still be at disadvantage to hit the invisible guy, but they would know what his location was. That makes sense :) Doesn't help most of us poor human-types though, unless your a werewolf or something!
 

Certainly you can close your eyes and try to find a hidden enemy by using your ears or nose. That's still a perception check and still allows you to "unhide" someone on success.
If there are two stones and an enemy that didn't wash himself for months, and it stinks behind one of the two stones, you know where the enemy is, without even using your eyes.
