Isn't that pretty much how many house rules are created?

Sure. But the important difference is that house rules are known by everyone, and ideally written down. They aren't at the mercy of just the GM remembering what he did the last time Situation X came up (and this can be all the worse when Situation X isn't really common).
 

I would generally go with "If you end up in a situation where story trumps rules or rules trumps story, you're probably not doing a very good job with one of them. Or both of them."
My usual feeling is that when that occurs, chances are there's a mismatch between the rules set chosen and what sort of stories you want to occur.
 





When its gotten to the point that I'd rather not even correct something that's wrong on a reply to my own comments, you know its bad.

Like, real bad.
