Permanent. It can be easily remedied with Remove Blindness/Deafness.

Although the PH doesnt specify I would have some issues with a 30gp mundane item having the same effect as a second level spell. Even the spell isn't truly permanent. The duration is permanent meaning it can be dispelled. You might want a duration for the thunderstone as a house rule for balance reasons.

It's no less permanent than a Thunderstone.

One of the DMs in my group did this, actually. I think he ruled it lasts 1 minute. Multiple Thunderstone effects are cumulative too. Sucks when four of those puppies drop at your feet. Not a bad ruling though.
