Except, of course, they actually say, "There is no such thing as an off-hand attack for a monk striking unarmed."
Not taking an actual position at all here... just an observation.
(Well, ok, and opinion too, I think that the rules state that you'd have to be using a weapon for the off hand attack. Of course, they don't state that an unarmed attack isn't a weapon... but that's circular arguing.)
The rules DO say that there's no such thing as an off-hand attack for a monk striking unarmed. And they also say that if you fire into a melee combat where one of the participants of that combat is friendly, you get a -4...
There's a feat to counter the second, feats are there for the express purpose of altering or adding onto base rules. Therefore a TWF feat creating an off-hand attack seems to fit in perfectly well.
