I think it probably would have been well served to have a question at the end ith something along the lines of: "Are there any other games that you play that are not D&D/Pathfinder?" With just an open reply field, because personally, I usually end up actually playing in games that are not.
My play group has several superhero games going on right now in the same shared universe. We've also played several Storyteller system games (WoD specifically), and just some system-free murder-mystery type games. Not to mention the Burning Wheel, Mouse Guard, Legend of the Five Rings, Serenity, Hollow Earth, Dark Heresy/ Rogue Trader/ Deathwatch, etc.
beyond that, I personally didn't have any issues with the survey itself, because my high fantasy games do tend to be D&D, but it would have been interesting to see, and I think would have helped your friend get a better idea as to the breadth of the participants that she's getting.
