1981. Greyhawk. My PC "Elliwovan Ellison", a cheap ass elven FMU at the time had 7 levels in magic user. He cast a fireball at yonder critter.

7d6. 42 points. All sixes, all at once. Read em and weep.

Been 25 years nearly since then - never had that happen again, ever.

I finished off with rolling 75% hits, wiping out all of his infantry and nearly all of his tanks - and lost a couple of infantry in total... Needless to say, Russia took over most of Europe that game...
Same game, I played Germany, attacked the russians. Attacked one territiory with tanks, infantry, and 6 planes. After the first round he shot 5 of them down with AA (ie, of 6 dice, he rolled 5 1s). Bye bye luftwaffe.

My best roll is a splendid tripleopenended in Rolemaster: roll was 371 before adding skill. I have witnessed almost quadrupleopenended 393 in RM

I've never managed to roll very well in D&D. Propably because my previous GMs have used expectation value in case where numerous dices should have been rolled - OD&D 20 d6 fireball caused 70 hits.
 

GURPS is 3d6 roll low for most things. The very fist check of the GURPS Traveller campaign I play in was my ex-crook picking a lock to avoid the opening (burger) bar brawl. I rolled 1+1+1.

I'm hoping it was symbolic.
 

OK, worst roll... not ever, but recently.

Last night, we were fighting undead. The fire-themed wizard says, "Undead burn nicely."

Then his first fireball (9d6) does 15 points of damage (4, 2, 2, 1, 1, 1, 1, 1, 1, 1). That's an average roll of 1.67. You can imagine the looks we gave him.

Second roll was a 38. (Average roll 4.22.) It's all good now.

I rolled a 18/100 for strength on my first ever character (first stat and first % dice ever rolled) but he was a dwarf and instead of making it 18/75 the DM made me re-roll :(
 

I've never had a particularly awesome roll, but my DM is insane with the dice. During one encounter, he rolled nothing but 19s and 20s on eight individual attacks in a row (including when confirming criticals). The attacks were distributed among the party, so luckily no-one was killed, but more than a few of us ended up on the ground, unconscious and bleeding.

The party couldn't believe it (nor could our DM), and we blamed it on his dice. But handing him our dice he continued to roll 17s and over. IIRC, at that point we decided to take a coffee break.
 

In an epic arena game, my lich cast a Grasping Hand at her psion enemy. The house rule for checks is that if you roll a 1 or 20, you reroll the die and deduct or add it to your total--this can happen more than once if you keep getting extreme results. Somehow, I ended up with a grapple check of 103. However, that wasn't as wildly improbable as the warlock who somehow got negative 32 for an initiative roll.
