It can plant epic suggestions while maintaining the fascination, such as "why don't you stand still while I massage you with my mouth".

Well I don't think I'd go for that - there wouldn't be much difference between that and domination. We could allow it to do more than a regular suggestion though I'm not quite decided what. Maybe allow multiple targets, give it a bonus to the DC, have it be an "extraordinary psychic power" that ignores magical defenses (so antimagic, mind blank etc are useless) or some combination of the three.

It does allow multiple targets as it's written. I like the ignoring mental defenses, but, really, I think the best boost would be letting him maintain it as a free action.

Well it doesn't seem quite like hypnotism if it requires no effort to maintain.

Maybe have it be a swift action or require a Concentration check if the Lizard takes serious damage?

Alternatively, have it suggest to one hypnotized victim as a free action or all hypno-victims as a standard action?

That would be fine. The main thing is that I don't want it to need an action to maintain the fascination effect. That would really handicap the Lizard Lord.

I'd rather it have to take some kind of action to keep on hypnotizing victims, but we could make it a Move or Swift action.

That makes it much less powerful than a gaze attack like the medusa's. Besides, the original monster says

Combat: In human form the lizard lord rarely uses weapons, preferring instead to depend on his strong hypnotism ability. Creatures meeting the lizard lord’s gaze must save vs. spell at a -3 penalty or become susceptible to the lord’s suggestions.

Why should we handicap that compared to a standard gaze attack?
