The same way it's hypothetically harder for a gunman to shoot you if you're moving his arm around.

There is a chance to hit the wrong target when using a ranged attack against a grappling target. (It was 25% in 3.0. Not sure about 3.5.)

In 3.5 if the attacker is attacking a target who is grappling (but attacker is not) with a ranged attack you roll randomly to see which grappling combatant you strike (which would be 50/50 I think) and the defender loses any Dexterity bonus to AC. If the attacker is attacking a target that he is grappling there is no mention of a chance that the might hit himself.
