Here's my system:
PC's get one hero point per level. Once used, they're gone.
A hero point may be used to:
1. Guarantee success on one die roll. Use of a hero point must be declared before a die is rolled. If used, the result is treated as a natural 20. For an attack roll, this automatically threatens a critical, but the critical must be conformed with a second roll.
2. Stabilize a dying character.
3. If a character is hit for enough damage that he/she goes from positive hp to dead, a hero point may be spent to restore the character to -1 hp, at which point the character is unconscious and dying.
Basically, it's a PC safety net, plus a chance to guarantee an extremely heroic act. Most of the hero points used have been to guarantee success on an attack roll against the BBEG.
