I guess if I am going to be critical of your scheme I might as well offer my own, so... Origin Types:
Immortal (all demons, devils, angels, primordials, and gods)
Elemental (the elementals, genies, salamanders, etc)
Fey (the problem with this is that it has a lot of crossover with Immortal, but...)
Undead (zombies, etc)
Construct (golems, warforged, etc)
Living/Animal (humans, animals, owlbears, etc)
Plant (treants, shambling mounds, etc)
Ooze (oozes, but shouldn't this count fungi too?)
Aberration (I don't like these, but they would be a type)
Form Types:
Humanoid (two arms, two legs, Humans, Orcs, Devils, classic Golems, genies, etc)
Beast (four legs: most land animals, Hellhounds, Unicorns, etc)
Avian (two legs, two wings: Birds, Rocs, Phoenix, etc)
Serpent (no limbs, long body: snakes and purple worms, etc)
Dragon (six limbs: true dragons, wyverns, pegasi?, Lammasu, etc)
Other (where could Beholders possibly fit in, anyways?)
The problem with this is that it might be getting a but too specific to be useful. Where would bugs and arachnids fit in? Do they deserve their own type? Where do a lot of the really bizarre Aberrations fit in? How does the many heads of a Hydra affect this? Do Pegasi really belong in the same type as ddragons? Would that mean humanoids with wings deserve their own type?
I'm not happy with my own creation, right now... At least I am fairly confident about the Origin types.
