Both the corporate world and the world of litigation have beaten that sort of accountability out of many. "Cover thine own butt" has become the 11th Commandment, if you don't want to pay an out of scale price. Fortunately, in Canada, we literally have a law that says an apology is not an admission of guilt.

Though that sort of thing also breeds the construct sometimes called the nonapology so people can try and have their cake and eat it too.
