Damage on a Miss is the One True Way! People who argue about it miss the point that it builds on the mechanics of the Original Edition! Sure, you had Save or Die situations, but there were also situations where a module would say, "if a player does X, they die, no save." Plenty of those!

Damage on a Miss just provides the minimum of mechanics to provide guidance for the application of this Random Damage! I should know, I DMed plenty of games back then, and let me tell you, hiding all of the players' bodies was extremely difficult! But if Gary said the player dies, then they're gonna die!
