Polymorph confusion!

OK, one of my players just got the polymorph self spell, and I'm trying to deal with it. Apparently there are numerous errata and different versions for polymorph running around. Can someone please clarify for me the current (final?) details about how polymorph works? Thanks a lot!

For example, does Polymorph Self allow you to switch forms every round? And is it a full-round action to change forms?

The most recent and final version of the polymorph line is listed in the Tome & Blood book for casters. They are quite specific in stating that it replaces the text fromt the PHB.

AFAIK, the errata for the PH and the spell in T&B are the same. The only difference is that they just reprinted the whole spell in T&B.

The PsiH has its own errata, and at least one of the three errata floating around has its own mistakes.

No, you can no longer change form every round.
