If a character gets reincarnated and her constitution changes as a result of her new race, do her hit points change to match what would normally be the effect of the change in the constitution's modifier?

It indeed says that "hit points remain unchanged." However, this would be taken out of context as reincarnation causes a level loss and it says everything remains unchanged. Level loss with everything unchanged would be a gross contradiction. If taken in the full context of the spell description, you'll see that it is a step-by-step process in which leaving the character intact is only the first step. The second step is to remove racial bonuses of old, the third step is to add racial bonuses of new, the third step is to apply a -1 Level/Hit Dice or -2 Constitution for first level characters.
