Step 1
You provoke an AoO from the target you are trying to garrote. If the AoO deals damage, then the garrote fails.

flat-footed targets don't normally get AoO's, right?

No i dont believe a FF'ee is entitled to a AoO. But if the target was not FF'ed, then it would provoke an AoO. This attack doesnt have to be stealthed.

Sure. The target would need to be unaware of you, and you would need to take the target into negative (not 0) hit points in a single strike. Additionally, this would require a Move Silently roll. I don't know what penalty I would assign to this roll. Probably the same penalty that applies to Hide rolls when sniping.

Of course, that's just my ruling. I don't know how it fits with the RAW.

You guys seem to be worried about the attack... the attack can be silent... it is the body falling to the floor or him dragging the body and such that the roll is needed for. If you kill them in one hit with a sneak attack and then you can make a MS roll to see if you can stop any other sounds.
