Eunuch warlock (OA): require to be male. Ex-male, in a way...
Battle-Maiden (OA): Require to be female.
Bladedancer of Eilistraee (F&P): require to be female.
That makes a couple (and a couple is two, not three or more; three or more is a vaudeville). I can't think of any other (official, that is).
But that's not so much a problem. In a world of magic, with all those Alter Self/Polymorph Self/Reincarnate/Cursed Items, sex and race are things that are little more than cosmetic. Once you've reached a certain level, if you've never changed of either (at least temporarily), you're extraordinary.
