The fighter utility, Boundless Endurance, lets you regenerate HP when bloodied, it is an stance so it lasts 5 minutes or till the end of the encounter..

So lets say an fighter has 40 hp, he gets hit and now has 18, he will regain 4 hp per round, he is now at 22 after the round..

- Does the regeneration continue even as he is not bloodied or does it stop as soon he is not bloodied?

Once you are no longer bloodied, therefore you no longer regenerate. If you a bloodied again while in the stance, you regenerate until you no longer are bloodied.
 

Also, if you become unconscious, you do not regenerate (even tho you are bloodied) because

A) Unconscious ends the stance immediately
B) Regeneration does not work when unconscious

Coulda sworn I read somewhere that unconsciousness conks out your powers, but you're right, I can't find it anywhere.

Regeneration generally stops working when you reach 0 hit points. See PHB page 293 and MM page 282.

This doesn't mean you lose the ability at that time, however. It only means the ability doesn't work when you're at 0 hp or lower.

(Yes, I'm aware the text says "until". If you want to run with the interpretation any creature with regeneration loses that ability permanently once it's been knocked unconscious even once in its life, feel free. I'm not going that way myself, though.)

For Boundless Endurance in particular, again the text isn't solid enough to prevent different interpretations. One interpretation I find reasonable is "when you're bloodied, you have regen; when you're not, you don't".

Then there's the question about how long does the power last, of course. Assuming BE is the only stance power you've got, this is simple: until the end of the encounter or for five minutes.

Perhaps I should add that I won't make these powers stop abruptly at the exact moment where the last enemy falls (which technically is the moment when the encounter is ended): I tell the fighter player he can assume the power lasts long enough to bring him up to just above bloodied before he needs to apply healing surges during the short rest.

PHB 267 - "The encounter ends when the [winning] side then takes a short rest or an extended rest."
PHB 278 - "Until the End of the Encounter: The effect ends when you take a rest (short or extended) or after 5 minutes."

So you're actually doing it right. ;)
