I thought the 300+ damage in a single round was strictly a 3e phenomenon.
My favorite was the Gauntlets of Ogre Power/Girdle of Storm Giant Strength/Hammer of Thunderbolts
Just the dwarven thrower. The hammer of thunderbolts was actually opposite - unusable by dwarves since it required a character height of over 6' (as well as requiring 18/01 or better). The HoT is +5 (requires girdle/gauntlet combo in addition otherwise it's +3) and kills giants it hits outright and does double damage otherwise, on top of the stunning everyone within 3". The DT on the other hand is +3, does double damage in general and does not kill outright, but does TRIPLE damage against giants.That was certainly my favourite potential combination. Did you have to be a dwarf to get the full benefits, or was that just the dwarven thrower?
12 lelel Crusader, with the aura of chaos stance [from 12l feat](re-roll max damage, indefinitely) and a needle or shuriken or a pixie dagger[all damage 1d1] of sure striking[bypass non-epic[+5] DR]. potion of true strike.
Damage:unlimited. (1d1 always rolls maximum. You can make one by the means of Mobius strip- you know 1 sided surface) The worst which can happen is your DM having your rollseveral thousand times.
