Flanking with yourself

I don't see how AOOs are relevant.

WXYZ

W=PC with chain, X=NPC, Y=NPC, Z=PC

The question is, does W flank with Z against Y?

'Threatened squares' are defined under the section on Attacks of Opportunity.

If there is a square into which you could not make an Attack of Opportunity, is it considered threatened?

I don't think that, just because the rule is mentioned under the broader title of AOO stuff it means that if you cannot make an AOO the rule no longer applies.

Otherwise all sorts of wonky rules results would come up. For example, once you make a single attack of opportunity you would no longer be able to flank for your allies turn. Do you play that if your character makes an AOO, he no longer flanks until his turn comes back around again (assuming you do not have combat reflexes)?

That's been argued in here before :)

You threaten a square into which you can make a melee attack. If it's not your turn, and you can't make an AoO, can you make a melee attack into any square?

This same line of thought came to me last night as I was reading the PHBII. I'd say, btRAW, yes, you flank with yourself.

But it just shows how badly thought out some of those PHBII feats are, IMO.
 

Not really it's a whole lot of feats to pull off, and it isn't going to happen very early in the game. Still countered with Uncanny Dodge, and larger than medium creatures.

There's a class in Iron Heroes that can flank with itself at higher levels due to movement.

Actually, every character in IH can do that, if he can move "around" the creature to flank with yourself. The Harrier probably has it the easiest, though...
