The character "recalls the majority of his former life and form" clearly means that there are some things from the past life and form that are not recalled. Is this like amnesia? Or is this like someone who has undergone weird psychotherapy to "recall past lives."
I think the loss of memory is the ingame reason behind the level loss.
 

Level loss isn't specific to Reincarnate, though--it's endemic to the process of returning from the dead. Which is to say, Raise Dead also causes you to lose a level, with no attendant memory loss.

I don't think the two are related.

Sorry to bring this off topic, but I've got a related question...

What happens if a creature combines natural attacks and manufactured weapons? Lizardfolk, for example:

5th level barbarian lizardfolk, +6 BAB, Multiattack, Greatclub, Bite

Is it:

A) +6/+1 Greatclub, +4 Bite?

or

B) +6 Greatclub, + 4 Bite?

Incorrect. What you are talking about would only apply if Bear weren't his natural form.

Of course, instead of waving his hands about like an idiot, he is bobbing his head and dancing around in a tutu. I'm not really sure which part would be worse, the tutu or not being able to cast spells. :(

Have you got a cite for that? I don't recall seeing that qualification before.

One thing:

1) The MM, under lizardfolk, says:

"2 claws +2 melee (or greatclub +2 melee), bite +0 melee"
