Also, clearly the SRD "code" is not a code; it is a set of parameters for acceptable code design. In my view, it should serve as the basis for a code designed collaboratively by GM and player. A paladin can't very well adhere to a code unique to him, unless he's the only known paladin; after all, how lawful would that be?

Agreed. For example, it's not necessarily illegal in Sharn (Eberron) - just restricted.

(However, it is illegal if it can do permanent damage, more than 1 point of Con damage, or more than 1d6 points to any other ability score.)

By the way - legal has absolutely nothing to do [directly] with lawful.
