No Trouble at All
Yeah, enough of the PC triumphs. What's the worst damage a monster or NPC has done to you?
Sometimes this shows up as a GM whoops. "Yeah, I knew that having a badguy with a keen greataxe with flaming burst might be bad, but I didn't think he'd kill you in one shot..."
The one I can think of is when I was playtesting an upcoming encounter for a short-run game of 4 10th level PCs. A hill giant with a ranger level was to ambush them. I originally gave the hill giant the Improved Critical feat, and a potion of haste. In my playtest the giant peeked over the ridge of the hill and threw three boulders at the flatfooted bard. Two confirmed crits. The bard was killed in one shot.
"Hmm... maybe I'll give him some other feat. He isn't meant to be a party killer."
