Somewhere in Faerun today, maybe somewhere in Illefarn, a Helm of Brilliance detonated in a brilliant explosion of Fireballs, Walls of Fire, and Prismatic Sprays, killing all but two of the party who just happened to still be around the corner and far enough away.

It took 27 years of gaming, but I finally had it happen. Unfortunately my character is at -2 HP, turned to stone, and on some random plane...

So new PC next time!

One of my old stories was when one of our party members threw his spear at an ogre mage chucking a necklace of fireballs. He aimed for the necklace around the ogre mage's neck and hit. A really spectacular explosion.

Yep. A 10th level Necromancer and his undead minions. This was the "Under Illefarn" module, I believe. I think that is the name the CK threw about.
