It helps to have the Dungeon issue where they printed the Shackled City adventure 'Foundation of Flame'. If I remember correctly, the template was ripped directly from that adventure. Some other things came from the Shackled City series, including things like traits (I can remember the conjoined twin and multiple arms thing from the various demodands) and mystical locations. Could be wrong though.
