I print out my own tokens using Powerpoint and pictures from various sources, chiefly Fiery Dragon's Counter Collection Digital and WOTC's art galleries and MtG cards.

Even if I'm in a game with a GM who doesn't use minis, I paint one for my character. In games that do use minis, I typically end up painting several different minis for my character as mini manufacturers produce new figures that suit my PC better than the one I had. In my dream game every piece of terrain and every NPC or monster would be represented by a beautifully painted mini. :)

I have always used miniatures in my games. Right from the Red Box Basic Rules set to 3.5 D&D. I started collecting and painting minis long before i even heard of RPGs. I don't paint much anymore but have 100's of the WotC minis and minis I have painted in the past.

Same here. I have a fair collection of DDM figs, but I also have some Reaper figs that I've painted. I especially like having a decent collection of PC figs so the players have a lot to choose from for their characters.

I have some unpainted Reaper figs, but many of them are in my paint box, so I don't really have access to them during play. I use little glass beads for things like rats or whatever, since I don't have any rat figs. I also just use markers to draw, but that's not as likely, or I might not even draw it out at all, if I think the combat is simple enough, or easy enough that it's not worth drawing out.

I've even been known to just handwave combat completely, if the encounter is way below their level.
 

I had to click "Other" because I use minis I painted myself, minis others have painted, plastic DDM minis, stuff from the toy store, counters, chits and even marks to represent things on the board. I primarily use plastic, but as "stuff I paint myself" increases, that may change.
