Are you saying that a double weapon like a quarterstaff is not allowed? If so, could you clarify your "game" a little more?

4th level human rogue with 1st level stats as follows: Str: 13, Dex 18, con 13, Int, 16, Wis, 13, Cha 15

Single opponent who did not move

Single melee weapon (not a double weapon)

No magic involved at all.

All three attacks were sneak attacks

I started the round flanking him

Again, why won't combat reflexes work?

You attack (1) normally.

Opponent pulls out a potion, provoking an AoO. You attack (2).

Opponent quaffs potion, provoking a second AoO. You attack (3).

QED.

Which means you had a Base Attack Bonus of +3, so one attack per round, barring anything else.

You aren't counting the partial action in the surprise round as part of your first round, are you?

For one thing, there's no partial action anymore. :)

For another, someone quizzing the Rules Forum as a "game" had better be balls-on accurate in his rules knowledge. So, the answer, if ever provided, had better not be due to a mistake in the rules or to some net-based or homebrew feat that no one's heard about. :p

Yes, although I did something to cause the AoO, not him.

All rules I used are from official 3.5 D&D rulebooks. This is supposed to be fun. If its not, Im sorry and Ill just give the answer and quit. Im not trying to be superior or smug or anything. The intent is for whoever figures it out to come up with their own question and try and stump everyone. Its 1005 my time, If no one has got it by 1230, Ill give away the answer.
 

So, you got three rapier attacks in a round. One from your base attack bonus, one AoO, and one from ?. Haste would be the common answer there, but you said no magic. A second AoO would be likely, but you said no Combat Reflexes. Cleave was a possibilty, but you said you were facing only a single opponent. I'd thought about improved trip, but you can't trip with a rapier, and you said all three attacks were sneak attacks, which means no trip attack. Short of some feat that I don't know about that gives you the flurry ability, I suspect a rules error.
