Ready action outside of combat

The EE in question won initiative, so it has its normal action (plus a partial action - maytheyrestinpeace - from the surprise round).

I missed the EE winning initiative. My bad.

The PCs fought a huge EE IMC last winter and boy were they surprised when it came right through the wall after they retreated down a narrow passage into the "safe" room. :D

Page 83 in the MM says that an earth elemental can "travel through solid ground or stone as easily as humans walk on the earth's surface."

Too bad they don't have tremorsense or they could attack while staying in the wall (or floor). Well, they still can, but they'll be doing it blind.

How does an EE figure out where it is going, if it doesn't have some sort of sense? I guess it just picks a direction...

Also, if the EE attacks from within a wall, would it be blind but have 100% cover (the surface of the wall being in front of it)?

Oh, and no ready actions out of combat. If you are ready for combat though, you can not be surprized. "I ready to shoot through the door when he opens it." while at the same time the mage on the other side "ready to chain lightning the adventurers when they open the door." Just roll initive, no surprise.
