Two weapon fighting - quick question

Ok ...

Character has 2 attacks per round and uses 2 weapons.

Stats are:

1st attack
Primary weapon: +14
Offhand weapon: +14

2nd attack:
Primary weapon: +9
Offhand weapon: +9

How many attacks per round does this character get?
I think it is 3.

Attack 1:
P (+14) + O (+14)

Attack 2:
P (+14)

No Offhand on additional attacks per round. Is this correct?

Actually I can't figure this out. ugh... Can someone please step me through this scenario:

9th level Fighter:

Str: 14 (+2)
Feats:
Two Weapon Fighting
Improved Two Weapon Fighting

Short sword in each hand

Base attack is: +9/+4
add STR = +11/+6

Start 2WF (-2/-2 because of feat and Offhand is Light)

1st attack= P: +9 O: +9
2nd attack=P: +4 (No offhand)

Ok, add Improved 2 Weapon Fighting
1st attack= P: +9 O: +9 O2: ????
2nd attack=P: +4 O2: ????

How does this work?
Do I get a 3rd Offhand attack and at what penality on 1st attack?
or
a 2nd Offhand attack on the 2nd attack at what penality?

Things I've seen make it look like this but I dont understand why:
1st attack= P: +9 O: +9
2nd attack=P: +4 O2: +4

I believe it's a -5 penality off of your base primary attack, that would make the additional I2WF feat be +4. +9-5=+4, if this is the case when do you get this attack, 1st or 2nd?

It would seem if it's -5 during the 2nd attack I should be at -1 (+4 - 5 = -1).

You almost have it right. Ignoring your Str bonus, it will look like this:
P:+9/+4
O: +9/+4

Without ITWF, you simply would not have that second off-hand attack (at +4). The values are otherwise identical. I think you are getting confused thinking you need to apply a -5 penalty to *all* your attacks, but that is not what the feat says. ITWF reads, "...you get a second attack with [off-hand weapon], albeit at a –5 penalty". So that penalty applies only to that additional attack, not all the attacks.
 

Here's my question.

If I am normally say:

1st attack: +10/+10
2nd attack +5

and then I add I2WF

It becomes:
1st attack: +10/+10
2nd attack +5/+5

Where does it say that the 2nd offhand attack come off of your 2nd attack, or is that assumed? And, does the -5 comes off of your Primary score?

Actually you've included the Str bonus (because it off sets the -2 penalty for fighting with 2 weapons and off-hand being light)

Total would go like this:

Primary: +9 (1d6+2)/ +4 (1d6+2)

Off hand: +9 (1d6+1)/ +4 (1d6+1)

And you must use a Full-attack action.

Err, why's the strength DAMAGE bonus less with the off hand weapon ?
