There is a compelling reason that the Refocus action was eliminated and it has to do with spells. In the Forgotten Realms (I believe it was Magic of Faerun to be exact) there was a spell that let you refocus your initiative as a 2nd level spell. I think it was called Renewed Focus. I don't remember whether it was automatically quickened, or whether you had to quicken it as normal, but it was sick.
After playing in a high level game with that spell, it was crazy good for our wizard for a time. Stupid reason you say? Yeah, perhaps. There are other 3rd party feats and mechanics that capitialized on the refocus action. Since you can accomplish the same thing by delaying in 3.5, there is no compelling reason to have a seperate action. This saves book space and confusion, and is one less thing for new players to learn.
I would like to see someone make a compelling argument as to why Refocus should remain in the game.
