Can someopne clarify two weapon fighting for me?

The PHB says one extra attack per round. So lets say I have two chars, one with a BAB of +6/+1 and another with +4.

Does that mean that they both get one extra attack with their offhand or that for each normal attack, they get an extra attack? This seemed really vague. Any clarification welcome. Cheers.

One extra attack with off-hand.

the guy with +6/+1 would have a two-weapon routine of
+4/-1, +4

the +4 char would have a routine of +2, +2

So regardless of how many attacks a character has, he only gets one extra with his off hand? Not one per main hand attack...

One extra attack with off-hand. Unless you have improved two-weapon fighting, then you get two with your off-hand. If you then get greater two-weapon fighting, you get three with your off-hand. If you take the Tempest PrC and get the supreme two-weapon fighting ability, you then get four with your off-hand.
