I'm the same. Actually, I think I'm pretty good at writing good descriptions. But on the fly--nope.

That's part of it, but keeping track of the spatial relationships in my head is a complete nonstarter. Ask me three successive rounds how far away a given thing is and I'll probably give you three different answers. And we don't tend to do loosey-goosey systems; 13th Age which we're doing now, is far looser than 99% of what we do (and even with it who can see what matters, so...)

Peanut Butter soup is a real dish out of African cultures. I’m not aware of any cuisine that puts pimento & olives in mashed potatoes, or lauds off-color ham. And flat out lying about roast beef vsprime rib? C’mon, son!

In today's session (Warhammer Fantasy RPG 4th Edition) we had a discussion about whether if a wizard creates a familiar in the form of a worm, and cuts that worm in half, shouldn't you now have two familiars?

When you played D&D before
Couldn't look you in the eye
You just like to roleplay
Your champion fighter makes me cry
You just like the fluff
And the lore of the campaign world
I wish I was special
Gonna make my character so special

But I'm a creep
I'm a powergamer
What the hell am I doin' here?
I don't belong here

I don't care if pun-pun is lame
I wanna have control
I’m gonna play the perfect game
It won’t matter what I roll
I want the DM to notice
When my PC is not around

My multiclassed optimized PC is special…

But I'm a creep
I'm a powergamer

Every once in a while I'd get an idea for an NPC and drop them in to the next game, but most often I made them up on the fly. Did it bite me in the ass? Yes, sometimes, but it was a lot less to remember before the game started and I tried to take good notes. There's tricks. For instance, when you drop in the NPC, think of a character from a TV show, movie, or video game, and emulate their mannerisms. I need a librarian? He's Giles from Buffy; now you've got vocal characteristics, attitudes, mannerisms, etc. After a while of use, the NPC develops their own character based on your needs, interaction with the players, and whatever is happening in the game world; but it's a fine trick to use.

Now I've got a Tiddlywiki for each campaign, stored in my Dropbox, that I access with TWCloud, so I can access them from anywhere, even my phone, and it's easy to edit. Works really good, and you can work on NPC background anytime and take live notes during the game. Highly recommended.

That's a whole different thing than the spatial/descriptive issues I was talking about to me.
