Sneak Attack Clarification Please

How does Sneak Attack work under RAW 3.5?

1. A rogue with 2 weapon fighting that attacks an opponent denied his dexterity bonus (either opponent is flat-footed or being flanked) adds sneak attack damage for each attack that hits, correct?

2. A rogue with iterative attacks that attacks an opponent denied his dexterity bonus adds sneak attack damage for each attack that hits, correct?

3. Sneak attack damage is considered precision damage and is not multiplied (for a critical hit, or any other condition that would multiply damage like a spear set to charge), correct?

Yes, Yes, and Yes.

For completeness' sake: an opponent isn't denied Dex to AC if they're flanked, although you do usually get +2 to hit them, and you can sneak attack them.
 

Yes, rogues can deal a tremendous amount of damage to living opponents (only) while flanking. Just beware the inevitably counter-attack. :)
 

Yes, yes, and yes.

Rogues are "p0wnt" when...

- Foes are immune to SA (elementals, undead, outsiders, anything at high levels, anything with fortification armor, etc)

- Foes don't die from the SA and can full attack you back
