I think @lowkey13 is right; chase this idea far enough, and the importance of making an attack roll (or any roll) diminishes to the point of being obsolete. I don't like "damage on a miss" for attack rolls, especially for martial characters and abilities, because risk vs. reward is a big part of my enjoyment.

Also, not everything needs to be the same as everything else. Attack rolls should be mechanically different from save throws, arcane arts should be mechanically different from martial arts, fighters shouldn't be rogues shouldn't be clerics shouldn't be wizards, etc.

That said: it's possible to add it to the game using the existing framework. I'd recommend doing it carefully, in very limited amounts and for very specific situations, but it's your game...do what you like.
