Does Spock know Romulans are related to Vulcans? He seemed surprised in TOS, but I don't know if that means anything.

I don't know that he was privy to the information that either Pike or La'an had. La'an's knowledge was specifically from a time travel incident, that she didn't relate to him.

Not sure how it ended up in the show BUT there is a massive local in joke in this episode if you're from Pittsburgh.

The recreation planet is called Permantti 3 or something similar and Ortegas mentions someone brought back some sandwiches with the fries inside the sandwich.

I think few if any Vulcans know the details of their Romulan kin. IIRC, the Romulans split during a chaotic and violent time in Vulcan history, and many historical records of the time are lost or incomplete.

In this episode, Spock attributed La'An's violent tendencies to her augment DNA, so he has no reason to question further. Pike and La'An are the only ones (so far) who know the truth about the Romulans, both due to time travel, so they cannot even share that intel with Starfleet, much less with Spock.

Yeah. While I think that La'an's ancestry is why the serum turned her into a Romulan instead of a Vulcan, I don't think Spock made that connection. His thinking would have stopped at "Her ancestry has made her more aggressive than a typical Vulcan."
